You must file your W-2s along with your Form 1040 tax return. Form W-2 is your “Wage and Tax Statement” from your employer. It indicates how much income you earned from the employer for a given tax year, and how much money was withheld from your income for taxes and other things. If you have lost your W-2 form, or even if you never received it, you are still obligated to file your tax return on time. You must make every effort to obtain another copy of your W-2 form. If your efforts prove fruitless, you must file Form 4852 along with your tax return.
